Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential.
Location:Santa Rosa California United States
Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management services.
- Provide hospice care.
- Collaborate with patients providers and other team members.
- Float between like units as needed.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Minimum of 2 years of hospital home health or hospice care experience.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Must be able to float between like units.
- RTO must be included at the time of submission.
Scheduling:
- Sunday through Thursday schedule.
- Specific shift type not provided.
- Block scheduling per manager discretion.
