We are seeking a dedicated Registered Nurse (RN) or Physician Assistant (PA) to join our Population Health Division. The ideal candidate will have experience or strong interest in value-based medicine high-risk patient management and addressing gaps in care. This role plays a key part in improving health outcomes for marginalized and underserved populations supporting our mission to deliver equitable high-quality care.
Key Responsibilities
- Monitor track and close gaps in care across value-based programs.
- Conduct outreach and follow-up for high-risk patients to support care compliance.
- Review medical records and clinical data to identify required preventive screenings chronic disease management needs and overdue health measures.
- Collaborate with providers care coordinators and the quality team to improve population health metrics and clinical performance outcomes.
- Support patient education on chronic disease preventive care and care plan adherence.
- Assist with the development and implementation of value-based care initiatives including workflows that improve quality scores and patient engagement.
- Participate in quality audits documentation reviews and reporting for payor-based value programs.
- Promote health equity by helping remove barriers to care for underserved patient groups.
Qualifications
- Active RN or PA license in New Jersey (required).
- Experience with value-based medicine quality measures or population health strongly preferred.
- Knowledge of HEDIS measures preventive screenings chronic disease management or care coordination is a plus.
- Strong communication assessment and patient engagement skills.
- Ability to work independently manage follow-ups and prioritize patient needs.
- A passion for improving health outcomes in marginalized communities.
