A highly reputable non-profit organization dedicated to delivering superior healthcare services to seniors is seeking an experienced MDS Coordinator (RN) for its Manchester New Hampshire location. This is an outstanding opportunity to make a meaningful impact in a beautiful 51-bed skilled nursing facility offering both short- and long-term care in a warm supportive environment.
About the Role
The MDS Coordinator is a specially trained licensed nurse responsible for assessing residents through the MDS tool to provide critical data on cognitive function nutrition behavior and this role youll help ensure accurate assessments and care plans that directly enhance residents quality of life.
Key Responsibilities
- Oversee and manage the full MDS process including assessments triggers and care planning
- Verify the accuracy and completion of all MDS submissions
- Maintain up-to-date PPS/Medicare and Medicaid/Private assessment schedules and distribute them to therapy dietary social services activities and nursing teams
- Stay current on Long-Term Care nursing practices procedures and state and federal regulations
Qualifications
- ASN required; BSN preferred
- Active RN license
- Minimum of 2 years of long-term care experience
