As a Hospice Medical Social Worker heres a big-picture view of what youll do:
- Provide home-based social services to patients dealing with end-of-life issues
- Work as part of a hospice team which will include an RN LPN CNA Spiritual Counselor and Hospice Aides focused on the patients plan of care and individual goals
- Monitor a patients psychosocial condition and identify social and emotional needs
- Conduct patient assessments and document progress
- Provide counseling community resource planning crisis intervention and advocacy
- Consult with family and caregivers on the patients plan of care
A few must-haves for Hospice Medical Social Workers:
- Masters degree in social work and licensed in the state of Louisiana
- Hospice social work experience required
- CPR certification valid drivers license active auto insurance and reliable transportation
- Knowledge of state and federal home health regulations
- Strong interpersonal skills; good communicator empathetic compassionate and resourceful
