Schedule:Monday Friday 8:00 a.m. 4:30 p.m.
Work Flexibility:Opportunity for aone-day hybrid remote scheduleafter 90 days of employment.
Role Overview
BJC HealthCare and Saint Lukes are seeking a dedicated Social Worker to join our Population Health team. You will be responsible for assessing the psychological social and financial factors impacting patient recovery. This role combines clinical knowledge with crisis intervention and discharge planning to support patients and families throughout their healthcare journey.
Key Responsibilities
- Clinical Assessment:Interview patients to identify social emotional cultural and financial needs.
- Case Management:Provide resources and support to caregivers and families; facilitate family therapy sessions as needed.
- Discharge & Transition:Develop transition plans manage post-acute referrals (including placements and transportation) and mitigate discharge delays.
- Multidisciplinary Collaboration:Partner with healthcare teams to ensure timely interventions and participate in program planning for the health system and community.
- Compliance:Maintain accurate and timely documentation within the Electronic Medical Record (EMR).
Qualifications
- Education:Masters Degree in Social Work (MSW) required.
- Licensure:Licensed Master Social Worker (LMSW) certification required.
