About the Role
We are seeking a compassionate and dedicated Community Care Assistant to join our Adult Social Care team within Day Opportunities. This role is essential in supporting individuals to live fulfilling lives by providing personal care emotional support and enabling independence through tailored activities and life skills.

Key Responsibilities

  • Act as a key worker helping to plan and review individual care plans
  • Provide personal and physical care respecting cultural and religious needs
  • Offer emotional support and practical assistance including escorting on journeys and helping with daily tasks
  • Encourage hobbies and activities that improve quality of life and independence
  • Work within health and safety guidelines and report any serious incidents promptly
  • Liaise with families staff and other agencies to ensure holistic care
  • Visit and support people in their own homes when required

About You

  • Some experience in a caring role (paid or unpaid)
  • Effective communication skills both verbal and written
  • Ability to provide personal care and implement enablement plans
  • Able to work independently and as part of a team
  • Willingness to travel extensively and work flexibly including weekends and bank holidays
  • Commitment to equality diversity and safeguarding practices
