The Mental Health Professional I works one on one with patients and facilitates group psychoeducation sessions. There is also typically an element of care coordination helping to coordinate services for patient to ensure all available community resources are utilized to the patients best advantage.

Treatment planning discharge planning and referrals:
Develop and maintain the patients treatment plan throughout the patients course of treatment with input from the care team
Keep patient family or others informed as to the patients progress throughout treatment as the patient desires.
Complete safety planning
Complete discharge planning
May lead psychoeducational groups
Coordinate patients social determinants of care needs
Practice de-escalation techniques
Provide patient information and make referrals
Understand Medicaid and other governmental benefit systems and help patients navigate through the public mental health system.
Help patients follow through with their health goals.
Coordinate medical and behavioral healthcare for patients
Assists patients with activities of daily living
Conducts assessments including but not limited to suicide screening and mental health assessments patient rights obtaining releases of information advanced directives etc.
May completes patient rounding in order to maintain a safe environment
Provides culturally and linguistically appropriate services
May transport patients
Documentation and Administrative Functions:
Maintains accurate concise and current clinical documentation.
Collaboration and Coordination
Participate in all required trainings and educational activities
Participate in department level meetings and huddles
Be responsive to patients in applicable communication mode
Education: Bachelors degree required. Mental Health related field prefered.
Experience: Previous healthcare & behavioral healthcare experience preferred.
License(s)/Certification(s): Drivers license if in an outpatient setting. May require class D Drivers license within 6 months of employment for outpatient setting. May be required to obtain BLS within six months of hire for certain sites.
Other: Use of usual and customary equipment used to perform essential functions of the position.
