Full-time position
Primary Purpose: To provide personal support to residents of the Thessalon First Nation that will assist in keeping them independent and living at home. As part of multi-disciplinary team the Personal Support Worker will provide services to the Home and Community Care clients who are recovering from illness injury and treatment; the frail elderly; persons with disabilities; and persons who present a need in any situation that jeopardises independence.
RESPONSIBILITIES/Duties
- Implement the client care plan at the times and periods indicated.
- Maintain and complete in-home client chart including flow sheets progress notes etc. reflective of direct and indirect client care.
- Completes and submits time sheets sign-in sheets and shift change requests to the Community Health Manager on a bi-weekly basis.
- Assist clients to develop plans of action while providing necessary supports
- Referring clients to other social services or assist clients in locating and utilizing community resources including legal medical and financial assistance housing and other services
- Meet with clients to assess their progress give support and discuss and difficulties or problems
- Assist clients with care by offering fluids snacks meal preparation
- Assist clients with mobility aids
- Ensure open communication with the client.
- Recognize and report all changes in the client condition to the Community Health Manager in order to ensure the care plan meets the ever-changing needs of the client.
- Has regular contact with the Community Health Manager as to the clients status and response to personal home care issues.
- Encourage clients involvement with community programming and activities.
- Provide comfort measures (make-up hair nail polish back rub).
- Assist with care by offering fluids snacks prepare meals accompany to washroom.
- Assist with bath time.
- Assist with proper use of mobility aids.
- Call the appropriate emergency services if required.
- To promote wellness and independence for all Home and Community Care clients in keeping with their physical mental and spiritual being while integrating the traditional and cultural beliefs of the client family and community.
- Establish and maintain open communication with clients.
- Accompany clients to medical appointments and/or programming as requested.
- Maintain the confidentiality of all client information.
- Participate in training as required.
MINIMUM QUALIFICATIONS:
- Must possess a Personal Support Worker certificate.
- Must possess a valid drivers license and have use of a reliable vehicle.
- Knowledge of Thessalon First Nation Community/or Aboriginal culture
- Will possess a current First Aid and CPR certificate.
- Must provide a CPIC and Vulnerable Sector reference check on an annual basis.
- Must complete a Physical Fitness Annual Physician Certification.
Salary: $23.00/hr
