Requirements:
- Active Registered Nurse License in the state of Ohio
- Tuberculosis (TB) screening/QuantiFERON/Chest X-Ray (within 1 year)
- Proof of U.S. work authorization (valid unexpired documents: government issued ID)
- Reliable transportation
Responsibilities:
- ConductingStart of Care (SOC)visits to evaluate medical physical and emotional needs and creating individualized care plans.
- Performingregular reassessmentsto monitor progress and adjust care plans as needed.
- Providingpatient and family educationon condition management medication adherence and preventive health measures.
- Collaborating withphysicians therapists social workers and familiesto ensure coordinated comprehensive care.
- Maintainingaccurate clinical documentation in compliance with Ohio Board of Nursing regulations and agency policies.
- Supervising and supportingCertified Home Health Aides (CHHAs)and other care team members.
