Licensed Practical Nurse (Every Other Weekend)
Job Type:Part Time
Location:In Person Fairfield County CT
Pay Range:$40- $50 an hour
Position Overview:
Health Care Connectors is hiring a skilledLicensed Practical Nurse (LPN)to deliver direct care to patients of all ages in a home and outpatient setting. The LPN will support daily clinical functions document care activities and participate in patient education and discharge planning.
Key Responsibilities:
- Provide hands-on nursing care including medication administration and vital sign monitoring
- Assist with specialized procedures and clinical assessments
- Coordinate care plans with the healthcare team
- Document patient care using EHR systems (e.g. WellSky HomeCare HomeBase)
- Educate patients and families on treatment plans and discharge instructions
Qualifications:
- Valid LPN license in Connecticut
- Experience with children and diverse clinical needs preferred
- Knowledge of sterile processing and infection control
- Effective communication and prioritization skills
