Admissions/SOC RN Home Health

A home health organization in Northbrook Illinois is seeking a full-time Admissions/Start of Care RN. This role provides compassionate care in patients homes coordinating care with healthcare professionals and ensuring compliance with healthcare regulations. The position offers a structured schedule potential additional earnings through on-call duties and eligibility for benefits.

Key Responsibilities

Provide admissions and start-of-care nursing services in patients homes
Coordinate care with interdisciplinary healthcare professionals
Ensure compliance with healthcare regulations
Cross-train as a Transitional Nurse Liaison in local hospitals while primary focus remains on field admissions

Schedule

Light full-time: three 12-hour shifts (9 a.m. 9 p.m. or 10 a.m. 10 p.m.) with alternating weekends
Two holidays per year plus 1.5 floating holidays
Travel within a multi-county territory required

Qualifications
Required

Active Illinois RN license
Minimum 1 year of hospice experience
Minimum 1 year of home health or related experience
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
High School diploma or GED
Comfortable with required travel

Preferred

RN license number available for credential verification
