Registered Nurse Case Manager Hospice Grayslake IL

A healthcare organization in Grayslake Illinois is seeking a compassionate Registered Nurse Case Manager (RNCM) to join their hospice care team. This full-time position offers a salary of approximately $80000 per year with additional compensation for on-call coverage and the opportunity to make a meaningful difference in the lives of patients and their families.

Key Responsibilities

Manage patient care in hospice settings
Coordinate care with healthcare teams
Provide leadership and support to nursing staff

Schedule

Monday Friday 8:00 a.m. 5:00 p.m. (start/end times may vary)
One 24-hour weekend shift per month (8:00 a.m. 5:00 p.m. scheduled visits 5:00 p.m. 8:00 a.m. on-call)
Two weekday on-call shifts per month (5:00 p.m. 8:00 a.m.)
Additional stipends provided for on-call duties
Dedicated after-hours RN coverage also in place

Travel

Local travel required across multiple counties

Qualifications
Required

Valid Registered Nurse (RN) license in Illinois (IL)
Minimum 1 year of hospice experience
Experience in home health care Epic Home Health or management preferred
High School/GED (minimum)
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Willingness to travel locally and fulfill on-call duties

Preferred

Previous experience with residential healthcare groups

Compensation

Estimated $80000 per year
