Registered Nurse Case Manager Hospice Kansas City MO

A healthcare organization in Kansas City Missouri is seeking a compassionate Registered Nurse Case Manager (RNCM) to join their hospice care team. This full-time position offers a salary of approximately $75000 per year with additional compensation for on-call coverage and the opportunity to provide meaningful care and coordination for patients and their families.

Key Responsibilities

Manage patient care in hospice settings
Coordinate care with healthcare teams
Provide leadership and support to nursing staff

Schedule

Monday Friday 8:00 a.m. 5:00 p.m. (start/end times may vary)
After-hours and weekend on-call duties rotate monthly averaging 5 shifts per month
- Weekday on-call: 5:00 p.m. 8:00 a.m.
- Weekend on-call: 8:00 a.m. 8:00 a.m.
Additional stipends provided for on-call coverage
2 holidays per year (1 summer 1 winter) and 3 floating holidays

Travel

Local travel required across multiple counties

Qualifications
Required

Valid Registered Nurse (RN) license in Missouri (MO)
Minimum 1 year of hospice experience
Experience in home health care Epic Home Health or management (1 years preferred)
High School/GED (minimum)
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Willingness to travel locally and fulfill on-call duties

Previous experience with residential healthcare groups
