A DAY IN THE LIFE OF A COMMUNITY LIVING SPECIALIST
In this position you will provide person-centered care coordination with older Minnesotans and caregivers over the phone and in-person. You will be providing options counseling advocacy resource location assistance and education. You will research home and community-based service options and assist clients as they navigate systems and comply with specific protocols and documentation requirements.
QUALIFICATIONS
2 years of experience with case management or care coordination in a long-term care hospital or community setting
- Compliance with all state licensure requirements
- Bachelors degree from an accredited college or university in a human services-related field. Social work and nursing degrees strongly preferred.
- Reliable transportation
- Heart for working with the elderly
Do you have excellent communication and interpersonal skills Are you highly organized and detail-oriented Do you have strong problem-solving skills Can you successfully work independently and alongside a team Are you great at time management If yes you may be perfect for this position!

This position is hybrid working in the community at home and in the office Monday - Friday 8:00 AM - 4:30 PM.
