How you will contribute
A Population Health Manager (RN) who excels in this role:
Manages the departments activities and resources to drive quality outcomes.
Reviews Electronic Medical Record (EMR) and payor registries to identify high risk patients.
Analyzes clinical data and identifies Hierarchical Condition Category (HCC) gaps.
Queries/communicates with providers/care managers to capture accurate Hierarchical Condition Category (HCC).
Assists in the evaluation of data collected to assure accuracy and reliability.
Prepares reports and performance reporting of quality data.
Oversees quality reporting to the insurance carriers.
Assists with orientation and training of personnel and students.
Participates and attends meetings as required and/ or assigned.
Performs other related duties as assigned or requested.
Prepares periodic reports as requested.
Assists Care Managers in making follow-up calls to patients.
What we are looking for
Applicants should have a state of Michigan RN license and possess an associate degree from an accredited nursing school (bachelors degree preferred).
