Duties: Job Summary:Ensures Physical Therapy services are administered and completed in a timely accurate and reliable manner. Provides physical therapy evaluations and treatments as well as associated clinical documentation. Maintains specific service delivery standards which address commitment to service customer satisfaction ongoing evaluation of customer expectations integration with quality process communication and interaction standards turn-around time and information processing standards corporate compliance requirements and physical amenities/environment of care. Job Responsibilities:-Assess patient strengths limitations and needs using standardized measurement prediction and evaluation tests.-Develop and implement individualized short- and long-term treatment goals based on patient assessments.-Administer and interpret clinical physical therapy services adjusting treatment plans as needed based on patient response.-Maintain accurate and timely patient treatment records documenting progress and changes in electronic health systems.-Ensure all documentation meets requirements of reimbursement and regulatory agencies completed within two business days.-Establish and maintain effective working relationships with patients families and interdisciplinary team members.-Participate in patient care conferences and collaborate with other healthcare professionals for coordinated care.-Provide education to patients and families regarding treatment plans safety and self-care techniques.-Participate in discharge planning and make appropriate referrals coordinating with vendors for durable medical equipment.-Supervise and provide direction to Physical Therapist Assistants during treatment sessions.-Maintain licensure requirements and pursue continuing education to stay current with professional standards.-Engage in departmental and organizational performance improvement initiatives to enhance quality of care.
Skills: Required Skills & Experience:-Previous experience in an approved medical facility. Preferred Skills & Experience:-Previous physical therapy experience in a hospital based setting.
Education: Required Education:-Graduate of an accredited school of physical therapy receiving applicable degree in physical therapy study with a bachelor s degree or equivalent. Preferred Education:-Master s or Doctorate Degree. Required Certifications & Licensure:-Possession of a current license or eligibility to be licensed as a Physical Therapist in the State of New York.-BLS Certification (AHA HSI or American Red Cross). Preferred Certifications & Licensure:-N/A
Schedule Notes: Shift: 0730-1630 M-F Main Location: 6 Wheeler St Greene NY Work setting is split between an onsite Occupational Medicine clinic and a manufacturing facility/warehouse setting (located across the street from office) Required: some level of experience beyond outpatient physical therapy; e.g experience in the form of ergonomic evaluation experience OSHA trainings or performing functional capacity/return to duty clearances Primary focus: injury prevention/early intervention returning workers back to work quicker work hardening routines Preferred experience: Experience in occupational medicine and industrial sectors OSHA experience ergonomics early symptom intervention and industrial settings experience working independently Typical Day: rounding warehouse floors and seeing patients connecting with HR business partners conducting follow ups with workers in clinic setting. Occasional travel to Binghamton &/or Syracuse for meetings required
