Description
As a Licensed Practical Nurse in Foster Care you will coordinate appropriate medical care dental care and mental health services nursing care/services to a youth and adolescent population with emotional behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.
Responsibilities:
- Communicate with medical director/providers regarding observation in clients health/behavior.
- Provide client and family education regarding medications and treatments.
- Collaborate with our staff regarding concerns and behaviors.
- Participate in Interdisciplinary Medication Review meetings.
- Nursing documentation using EHR.
- Update discharge planning needs.
- See foster children in foster homes as needed.
- Other duties as assigned within scope of practice and based on agency needs.
Requirements and Education:
- LPN Licensed in New York State.
- 1 to 2 years of experience.
- Experience working with youth and adolescent population desired.
- Must pass physical.
- Must have a valid NYS drivers license and reliable transportation.
