The successful applicant may be eligible for an annual full-time incentive of up to $2000 each year until 2025 as per MOU #31 of the current MNU collective agreement.
The Access Coordinator is a pivotal member of the cardiac care team and utilizes advanced cardiac skills and knowledge to: coordinate information; facilitate the movement of patients through the admission transfer and discharge process; liaise between internal and external programs and external programs and agencies on matters related to patient movement; and assist referring physicians to access cardiac care. The incumbent facilitates physician to physician communication and manages the waitlist for cardiac care. This includes working in conjunction with the patient cardiac specialist(s) referring physician(s) and hospital staff to monitor patients either at home or in hospital and triaging inpatients and outpatients by acuity level to ensure appropriate and timely access to cardiac care. The Access Coordinator is responsible for maintaining data bases and reporting statistical data. The incumbent provides patients and their families with information and support throughout their journey in receiving cardiac care. The Access Coordinator utilizes advanced skills and knowledge and performs at a high level of independence as a clinical practitioner leader consultant and educator. Practices are in accordance with employer policy and professional standards.
