This recruitment is for one (1) full-time permanent Forms & Records Analyst 2 (FRA 2) position withinthe Office of Customer Service in the Health Services Quality Assurance Division. This position requires the incumbent to perform their work duties on-site at our Tumwater WA DOH Office.

The Health Systems Quality Assurances (HSQA) top priority is to protect and improve the health of people in Washington State. We do this by helping to prevent injuries develop health systems and improve patient safety. We work closely with communities and local health partners to build strong health systems and prevention programs. This helps ensure that everyone has access to good healthcare and emergency medical services.


TheOffice of Customer Service (OCS)is the primary contact for the public healthcare providers facilities emergency medical services and many other customers. We deliver these services in-person over the phone and on the of Customer Service balances patient safety with access to care in its credentialing and customer service activities to ensure that Washingtons health care system works for all.Our efforts result in everyone getting the quality care they need when and where they need it.


Key Responsibilities Include

  • Receives analyzes interprets and matches sorts collates scans indexes and processes health provider Emergency Medical Service (EMS) and facilities credentialing-related documentation files records and application forms in a timely manner.
  • Accesses and organize files in a paper-based environment including retrieving files from a variety of locations. Must move or transport boxes of files weighing up to 30 pounds.
  • Utilize manual electronic and/or automated systems to input data and navigating a database creating and modifying electronic documents and using email and internet programs.
  • Integrates hardcopy documents and other forms delivered through mail or other means of receipt into scanning workflows for digitizing for further application processing. Updates information files and maintains accuracy of HSQA credentialing information and records.
  • Accesses and organizes files in a paper-based environment including retrieving files from a variety of locations including moving or transporting boxes of files weighing up to 30 pounds.
  • Maintains records systems and may contribute to the design of alternative electronic systems with related indices to meet specialized credentialing application filing requirements.
  • Provides consultation and technical assistance to credentialing staff on matters of records access interpretation gap analysis and system troubleshooting.


This position supports the Department of Healths (DOH) mission to protect and improve the health of people in Washington State by performing activities within a section responsible for credentialing health care providers emergency medical services (EMS) and licensing facilities. This position is responsible for performing work in support of credentialing within the centralized Credentialing section of the Customer Service Office within the DOH Health Systems Quality Assurance Division. Following guidelines this position performs routine organizing sorting scanning filing and indexing activities related to credentialing processes.

Location and Flexibility:Join our onsite team to collaborate directly with colleagues and be part of our dynamic workplace environment. This position requires working onsite and in-person at our Tumwater Washington DOH Office 5 days per week.



Required Qualifications

You must meet the option provided and any additional criteria listed. Experience may have been gained through paid or unpaid activities. Please ensure any relevant experience defined below is outlined in your cover letter resume and/or applicant profile.


  • Two (2) or more years of office clerical or administrative work experience; and
  • One (1) or more years of experience providing customer service in a professional office setting; and
  • Six or more months of computer software skills and experience in the following:
    • Software such as Windows file indexing folder creation and management.
    • Word processing software such as Microsoft Word to create format edit preview print and save documents. Use standard functions to select edit copy paste format and spell check text.
    • Electronic communications and calendar programs such as Outlook - to compose send and respond to electronic mail and coordinate schedules and schedule meetings.
    • Database experience using database software or web-based applications to update and/or maintain records.
    • Internet using the internet as a research tool to find and download information.


As well as the following knowledge skills abilities and competencies:

  • Ability to learn retain and apply new information regarding procedures rules policies and requirements for credentialing multiple health care professionals.
  • Ability and willingness to work within a secure work environment with highly sensitive and confidential materials.
  • Ability to work independently with little supervision.


Preferred/Desired Qualifications:

  • Associates degree or higher; OR two (2) or more years of successful completion of college-level coursework (quarter hours or semester equivalent).

  • Six (6) or more months experience using software programs to convert and index documents after scanning such as but not limited to Adobe Acrobat or Nuance.

  • Two (2) or more years of analytical experience reviewing documents and taking appropriate action based on an analysis of the information.

  • Two (2) or more years of experience resolving customer inquiries problems or complaints in a high production environment.

  • Six (6) or more months of credentialing experience processing healthcare professionals or facilities licensing applications.

  • One (1) or more years of experience reviewing work for quality and accuracy.
