Responsibilities
- Deliver high-quality services to care leavers ensuring successful transitions into adulthood.
- Prepare review and implement Pathway Plans in collaboration with the supervising manager ensuring parallel planning where applicable.
- Conduct human rights assessments to uphold and promote the rights of young people.
- Coordinate service provision and ensure care leavers access available services.
- Maintain accurate records of all contacts interventions and services on the care leavers electronic case file.
- Work holistically with care leavers and their families during transitions or transfers to additional services.
Essential Experience Required
- Minimum of 2 years experience working with young people in a supportive role within a voluntary or statutory setting.
- Proven ability to empower and support a person-centred approach in developing planning and reviewing support plans.
Essential Qualifications Required
- Level 3 qualification or above in Social Care Youth Work Psychology Education or a related field (or equivalent experience).
Additional Information
- Working hours: 36 hours per week
- The role requires a DBS check.
