Support the health and wellbeing of Australian First Nations people accessing our programs

2-days a week currently funded until 30th June 2026

Primarily based at our West Heidelberg site.

Will involve mix of centre-based and outreach with some group work

YOUR NEW ROLE:

Holstep Health is seeking to employ an enthusiastic and experienced person as an Aboriginal Wellbeing Liaison to work within our Aboriginal Health Team.

The Aboriginal Health Team at Holstep Health includes a Counsellor Social Worker Community Nurse Aboriginal Health Liaison as well as the Aboriginal Wellbeing Liasion who work together to provide flexible tailored responses to meet the needs of our clients in a timely and appropriate way supporting recovery and self-determination and recognising the episodic nature of mental health concerns.

The Aboriginal Wellbeing Liaison works alongside the counsellor in our Aboriginal funded programs to provide culturally appropriate support and help improve the health and wellbeing of people clients accessing our programs with the aim of making community based mental health care accessible and culturally relevant for Australian First Nations people.

The role also works closely with other internal Aboriginal-specific programs community support and mental health programs as well as Aboriginal Controlled Organisations and other service providers to support clients in their health and wellbeing needs

The role will provide both onsite and outreach services for clients of the program as well as supporting therapeutic and cultural support groups that are part of the Psychological Support Strategies program. The role may be delivered in a number of settings including within the health service in the home and other community spaces. This role sits within the Aboriginal Health Team but also within a broader Community Supports Team that has social workers financial counsellors and community nurses.

YOU WILL BE RESPONSIBLE FOR:

To support Aboriginal and/or Torres Strait Islander clients overcome barriers to accessing services that support their independence wellbeing and empowerment

To liaise with internal and external health services Aboriginal Community Controlled Organisations and other community services to support a holistic service delivery for Australian First Nations clients

Organise and support therapeutic and cultural support groups that are part of the overall psychological support strategies program

To work with staff from the Aboriginal Health funded programs and other Holstep Health workers to deliver culturally safe and appropriate services

Maintain client records and report data in line with agency procedures and funding guidelines

Identify barriers that prohibit Aboriginal clients from accessing Holstep Health services and advocate for changes to practice to build better access

Participate in team meetings as identified by your direct line manager and maintain effective working relationships with all staff and regularly participate in team and organisational activities

WHAT YOU NEED TO SUCCEED:

Qualified Health Workers preferred including Aboriginal Health Workers or similar with relevant experience

At least two years experience working with Australian First Nations communities

Demonstrated ability to liaise with Aboriginal and/or Torres Strait Islander clients and other professionals to ensure clients can access the health and wellbeing support services they require

Deep understanding of Aboriginal and Torres Strait Islander communities and their experiences of health systems in particular psychological services and supports

Well-developed skills in supporting cultural and therapeutic group activities

Demonstrated ability to work collaboratively with a range of health professionals including specialists GPS nurses counsellors and social workers

Necessary skills to maintain client records (including confidentiality) and keep all relevant program data associated with the funding requirements of the program

Knowledge and understanding of Aboriginal and Torres Strait Islander communities and their experiences of health systems

An understanding of mental health and wellbeing issues within First Nations communities

An understanding and commitment to the Social Model of Health particularly in relation to First Nations communities

Aboriginal and / or Torres Strait Islander status highly desired
