Position Summary

The Direct Support Professional (DSP) Home and Community provides individualized support to people with disabilities focusing on self-help socialization and independent living skills. Services are provided in the individuals home and throughout their community aligned with person-centered goals and outcomes.

Key Responsibilities

• Support individuals in achieving goals through person-centered approaches.

• Maintain a balance between health and safety and individual independence.

• Assist with home and community-based activities that promote growth and engagement.

• Maintain professional relationships with families and team members.

• Attend team meetings and communicate regularly with supervisors.

• Complete all required trainings and comply with incident reporting standards.

• Use a smart device for documentation and communication as needed.

Qualifications

• High school diploma or GED required.

• Minimum of one year of experience providing direct support to individuals with disabilities.

• Valid drivers license and properly insured personal vehicle (mileage reimbursement provided).

• Must pass PA Act 33/34 and FBI clearances.

• Must have a smart device for work-related use.

• First Aid/CPR certification required within six months of start date.
