- Support HWD by managing Assisted Living team members.
- Schedule Assisted Living team members.
- Participate in Care Plan meetings with families and residents and manage ISPs as resident needs change.
- Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.
- Participate in weekly at risk meetings with HWD ED MCC/ MCD to ensure leadership team is aware of all resident needs.
- Review and implement all necessary processes for optimal operational efficiency such as- shower schedules laundry schedules caregiver assignment sheets etc.
Qualifications :
Level of Formal Education: HS or GED
Area of Study: General
Years of Experience: 3-5 years caregiver experience preferred
Type of Experience: CAN experience preferred scheduling communicating with families
Special Certifications: Prefer CNA
Language Skills: Acceptable fluency in English in order to perform job duties and speak read write and communicate with supervisors coworkers residents and applicable third parties.
Technical Competencies: Basic computer skills Excel for scheduling
Skills and Ability:
- Need to be organized flexible and even tempered.
- Ability to communicate with families frontline team and community management team to ensure optimal operational success.
Personal Attributes: Calm personality that possess the ability handle stressful situations with an easy demeanor ability to multi task and follow through and self-motivated.
