Overview:
We are looking for a compassionate and organizedOnboarding Specialistto join our home health care this role you will welcome new patients ensure they understand our services and provide a warm and supportive introduction to their care journey. Youll be the first point of contact for new patients and familieshelping them feel comfortable informed and confident as they begin home health services.
Basic Function:
- Conduct welcome calls to new patients and introduce them to our home health services.
- Verify patient information and confirm their understanding of the care process.
- Address initial questions or concerns from patients and family members.
- Provide an overview of what to expect during their home health care experience.
- Ensure all onboarding documentation is completed accurately and in a timely manner.
- Communicate relevant patient details to nursing and administrative teams.
- Follow up with patients as needed to enhance their onboarding experience.
- Maintain detailed and accurate records of patient interactions.
- Coordinate care efficiently with ancillary companies and departments.
Requirements:
- High school diploma or equivalent required; associate or bachelors degree preferred.
- Previous experience in home health healthcare administration or patient services preferred.
- Excellent verbal and written communication skills.
- Compassionate patient-focused and professional demeanor.
- Strong organizational skills and attention to detail.
- Proficient in Microsoft Office and comfortable learning new software systems.
- Ability to work both independently and as part of a team in a fast-paced environment.
