Job Description

Summary:
The Registered Nurse Case Manager is responsible for managing the clinical and financial outcomes of patient care through effective coordination discharge planning and utilization management. This position ensures patients receive appropriate high-quality care across the continuum while also supporting compliance with hospital state and federal regulations. The RN Case Manager collaborates with physicians nurses social workers and insurance providers to ensure safe and timely transitions of care.

Responsibilities:

Coordinate and manage the care of assigned patients to achieve quality outcomes and cost-effective resource utilization.
Perform comprehensive assessments to identify discharge needs and develop individualized plans of care.
Collaborate with the interdisciplinary team to ensure smooth transitions from acute care to post-acute services (home health SNF hospice etc.).
Monitor and document medical necessity level of care and length of stay using InterQual criteria and evidence-based guidelines.
Communicate with insurance companies to obtain prior authorizations review benefits and manage appeals/denials as needed.
Serve as a liaison between patients families and healthcare providers to promote understanding of care plans.
Educate patients and families about post-discharge care available community resources and follow-up requirements.
Ensure compliance with CMS DRG HIPAA and other regulatory standards.
Participate in quality improvement and case management performance initiatives.

Key Requirements:

Minimum 2 years of acute care case management/discharge planning experience required.
Travel experience required; first-time travelers may be screened.
Must have strong clinical knowledge in discharge planning utilization review and care coordination.
Must be proficient in Epic charting system.
CA RN license required (pending accepted if issued prior to start).
Must be available every other weekend and for holidays.
RTO Restrictions: No more than 7 days per contract.
Floating: Not required.
Ratios: Days 1:20.

Qualifications

Education:

Graduate of an accredited School of Nursing (ADN required; BSN preferred).

Experience:

Minimum 2 years of recent acute hospital case management/discharge planning experience.
Previous experience in utilization review and interdisciplinary care coordination highly preferred.
Familiarity with CMS regulations DRG methodology and payer documentation requirements.

License:

Active California RN license (pending accepted if active prior to start).
BLS certification required.

Skills:

Clinical & Case Management Skills: Discharge planning needs assessment plan of care development care coordination utilization review.
Regulatory Knowledge: CMS DRG HIPAA Workers Compensation processes.
Analytical & Documentation: InterQual criteria admission criteria medical necessity review concurrent chart review.
Communication & Collaboration: Liaison with patients families payers and interdisciplinary teams.
Preferred Exposure: Behavioral Health Home Health Hospice Skilled Nursing or Insurance-based case management.
