Job Description

Summary:
The Inpatient Pharmacist is responsible for providing safe effective and evidence-based medication therapy for patients in an acute hospital setting. This role includes compounding preparing verifying and dispensing medications as well as providing clinical consultation to physicians and nurses. The pharmacist ensures compliance with all state and federal regulations hospital policies and best practice standards for medication safety and patient care.

Responsibilities:

  • Review and verify medication orders for accuracy appropriate dosing and potential drug interactions.

  • Compound and prepare sterile and non-sterile products including IV admixtures antibiotics and hazardous drugs per USP 797/800 standards.

  • Dispense and monitor medications ensuring therapeutic effectiveness and patient safety.

  • Provide clinical support to medical and nursing staff on medication use dosing adjustments and adverse drug reactions.

  • Conduct therapeutic drug monitoring including Warfarin Vancomycin Aminoglycosides and other high-alert medications.

  • Adjust medication therapy for renal geriatric and pediatric populations as needed.

  • Document interventions dosing recommendations and clinical notes accurately in Epic.

  • Oversee Pyxis or other automated medication dispensing systems.

  • Participate in interdisciplinary rounds and assist with formulary management and cost-effective medication use.

  • Adhere to aseptic techniques infection control and institutional safety procedures.

  • Maintain knowledge of hospital protocols drug shortages and formulary updates.

Key Requirements:

  • Experience: Minimum 1 year of recent inpatient hospital pharmacy experience required.

  • Charting System: Epic required.

  • Environment: Experience in acute hospital or compounding pharmacy preferred.

  • Travel Experience: Not required; must have strong inpatient background.

  • Certifications:

    • PharmD or RPh (one required).

    • State Pharmacist License (active and unrestricted).

    • IV Certification required.

    • Chemo/Hazardous Drug Certification preferred.

  • Float: May be required to float between like units as needed.

  • Weekends: Required (rotational schedule).

  • Pending License: Not accepted.

  • Locals: Accepted (same rate applies).

  • RTO: Must be included at time of submission.

  • Block Scheduling: Per manager discretion.

Qualifications

Education:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy (RPh) from an accredited college or university.

Experience:

  • Minimum 1 year of inpatient hospital pharmacy experience required.

  • Experience with compounding sterile preparations and IV admixture required.

  • Previous experience in Trauma Level II or community hospital setting preferred.

License:

  • Active unrestricted State Pharmacist License (no pending accepted).

  • IV Certification required.

  • Chemo/Hazardous Drug Certification preferred.

  • BLS certification preferred (per facility policy).

Skills:

  • Compounding and Preparation: Intravenous preparation IV push IV piggyback antibiotics and hazardous drugs.

  • Clinical Dosing Expertise: Anticoagulants cardiac medications renal and geriatric dosing pharmacokinetics and therapeutic monitoring.

  • Medication Systems: Proficiency with Pyxis or equivalent dispensing systems.

  • Technical Knowledge: Familiarity with Epic charting USP 797/800 guidelines and aseptic technique.

  • Professional Attributes: Strong attention to detail communication teamwork and ability to function in fast-paced acute environments.
