Key Responsibilities:
Conduct comprehensive speech language cognitive and swallowing assessments.
Develop individualized treatment plans and deliver therapy using evidence-based practices.
Maintain accurate and compliant documentation for SNF standards.
Collaborate with nurses rehab staff physicians and families to support resident progress.
Participate in care planning and interdisciplinary meetings.
Qualifications:
Masters degree in Speech-Language Pathology from an accredited program.
Active SLP license in the state of Massachusetts.
Minimum 1 year of experience in a Skilled Nursing Facility (SNF) long-term care or adult/geriatric rehab setting.
Strong communication and interpersonal skills.
Experience with dysphagia and cognitive-communication disorders preferred.
