SHIFT: 7:30am-4:00pm Mon-Fri Varied As Needed - up to 16 hours a week
Functions as a Pharmacist as defined by the Iowa Board of Pharmacy Examiners. Responsible for providing all aspects of pharmaceutical care including clinical and distrubutive tasks for patients served within UnityPoint Health Grinnell Regional Medical Center as part of the UnityPoint Health Des Moines Region. Acts as a member of the health care team to help patients and clinical staff achieve clinical outcomes and monitor progress toward common goals. Performs assignments and projects as directed by the Pharmacy Management Team. Supervises Pharmacy Technicians during the absence of formal departmental leadership.

Operational Functions
Oversees technical and support staff in the absence of administrative personnel
Verifies the accuracy of medication stock in automated dispensing cabinets
Reviews the work of other pharmacy personnel to ensure the appropriate dispensing of medications per the prescribers order in accordance with laws and regulations
Serves as a drug information resource for the health care team patients and community members
Inspects pharmaceutical supplies storage and utilization on assigned patient care areas
Maintains a neat clean orderly and secure department and work area
Clinical Functions
Reviews all antibiotic orders looking for appropriateness of drug selection schedule route indication length of therapy drug to drug interactions allergies and bug/drug mismatches
Clinical documentation of outcomes and reviews using electronic health records
Practices pharmaceutical care activities: reviews medication orders to assess the appropriateness of drug selection for the patient based on patient specific parameters (e.g. renal and hepatic function co-morbidities patient-specific medication compliance issues etc) evaluates medication regimens for appropriate dosing schedule route contraindications and potential drug interactions; contacts the prescriber as necessary for clarification or to recommend appropriate therapy or dosage changes
In accordance with established policies procedures and protocols specific activities may include but are not limited to:
Dosing of medications using the principles of pharmacokinetics
Assessment of select IV medications to determine if oral administration is appropriate
Actively supports and participates in medication reconciliation
Evaluation and interpretation of culture and susceptibility reports for antimicrobial recommendations
Documents pharmacist interventions in the electronic health record
OTHER FUNCTIONS:
Situations of a critical nature may require an individual to perform multiple tasks until additional support can be obtained
Functions other than described above may be needed occasionally required to provide optimal patient care
Education:
B.S. Pharmacy with equivalent clinical experience
Doctor of Pharmacy (Pharm.D.) preferred
Completion of an ASHP accredited PGY-1 pharmacy residency preferred
Experience:
Must be able to proficiently read write and speak English
Proficiency in navigating the electronic medical record
One or more years of hospital pharmacy experience preferred
License(s)/Certification(s):
Iowa Pharmacist license in good standing or eligible for licensure within six months of employment; must be directly supervised by a licensed pharmacist until Iowa Pharmacist license is obtained
Must maintain current license to practice pharmacy in Iowa and be in good standing with the Iowa Board of Pharmacy Examiners
Current Basic Life Support (BLS) certification or completion within three months of hire.
Board Certification preferred
Valid drivers license when driving any vehicle for work-related reasons
