About the Role

Based within the newly established Lower Mid-North Coast (LMNC) Hub this role offers a unique opportunity to provide specialist coordination and leadership in palliative and end-of-life care. Youll work collaboratively with a dedicated multidisciplinary team - including nursing allied health medical specialists volunteers and GPs - to ensure patients and families are supported through every stage of their journey in the environment that feels right for them.

Each day brings variety and meaning - whether youre leading care planning discussions supporting colleagues to deliver safe and compassionate practice or guiding families through some of their most vulnerable moments. This is an opportunity to shape integrated person-centred care across both home and hospital settings.

About You

Youre calm under pressure kind under challenge and confident in your clinical leadership. Youll bring advanced skills in palliative and end-of-life care along with the ability to prioritise collaborate and communicate with empathy. A reflective practitioner and a natural team player you value connection trust and the small moments that make the biggest difference.

Essential:

Current AHPRA registration as a Registered Nurse

Post-registration qualification in Palliative Care

Minimum three years experience in the specialty

Current drivers licence and ability to travel across the LMNC region

Orientation and ongoing support include guidance from your Nurse Unit Manager collaboration with the CNC2 (Integrated Care Services) and participation in peer and case discussions within the multidisciplinary team. Youll also have access to development through the HNELHD Palliative Care Stream regular manager catch-ups and professional supervision designed to sustain both your skill and your wellbeing.
