Position Summary

Under the direction of the Team Lead Coordinator and the EUTC Program Manager the Fresh Start Support Worker is a member of the EUTC Reaching Home - Fresh Start team and will work together to provide services in a flexible compassionate and responsible manner to participants of the Fresh Start Project. The Case Support worker is to follow the Intensive Case Management model that utilizes the housing first philosophy based on flexible and community-based support promotion of stability recovery and community integration.

Key Responsibilities

  • Collaborate with the Follow-up Support workers in the formulation of a Service Plan that follows the housing first principles that is reviewed and modified with the participant on a regular basis.
  • Collaborate with internal project support including the Service Coordinator to help and assist the Follow-Up Support Workers in Participants action plans to find housing maintain stable housing of their choice.
  • Assist with outreach case management counseling advocacy and other needed services to participants in any environment including: the streets shelters prisons hospitals apartments office etc.
  • Educate inform and advocate for participants regarding benefits and entitlements i.e. rights and responsibilities of tenant and landlord employment and income assistance employment insurance and food vouchers.
  • Provide life skills workshops and information sessions on various topics such as substance abuse harm reduction safe sex domestic violence employment healthy living wellness etc.
  • Coordinate/monitor referrals to community services and advocate participant participation in them.
  • Educate inform and advocate for participants regarding benefits and entitlements.
  • Provide up to date information on community programs and services that the participant may be interested in partaking in - i.e. employment programs community sweats etc.
  • Conduct monthly apartment inspections and document findings on inspection form.
  • Assist with apartment condition report any building maintenance issues to submit through HIFIS.
  • Ensure smoke detectors and carbon monoxide detectors are in working order in each apartment.
  • Maintain written and computerized records compile reports and complete other program documentation (including case notes HIFIS statistics letters etc.).
  • Provide monthly reports of activities to team lead coordinator and Program Manager.
  • Other duties as assigned.

Qualifications

Education & Experience

  • A minimum of an undergraduate degree or college diploma in the field of social service or a combination of related education and training will be considered.
  • Knowledgeable of the Personal Health Information Act (PHIA) and Service Priority Decision Assistance Tool (SPDAT).
  • Demonstrated knowledge of Housing First concept.
  • Patience creativity flexibility compassion and sensitivity to persons who have experienced the child welfare and criminal justice system.
  • Skills in assessment case management and advocacy.
  • Excellent interpersonal and communication skills and the ability to work independently and co- operatively as a part of an interdisciplinary team are required.
  • Knowledge of Winnipeg services and resources in the areas of supportive housing mental health counseling addictions child welfare and justice systems and First Nation culture.
  • Proficient in using Microsoft Word Excel Power Point and Outlook.
  • Must be dependable reliable and work as a team player.
  • Able to work flexible hours and travel upon request.
  • Must have a valid Class 5 drivers license and a reliable vehicle.
  • Must have a current satisfactory Criminal Record Check Clear Adult Abuse and Child Abuse Checks.
