Job Summary

Resthaven is calling for an Expression of Interest for the position ofSocial Coordinator. The Social Coordinatorwill work with individuals community teams and the Wellness and Social Lead to design and deliver a suite of wellness and social programs that enhance the lived experience for older people in the position is offered as a Full-Time permanent role.

Role Responsibilities:

Contribute to a goal orientated service model that focuses on the delivery of person-centred services and care
Collaborate with Care Partners to ensure person-centred services are delivered
Ensure compliance with all program guidelines and expectations
Working with individuals offer social and wellbeing programs that enhance engagement wellness and a positive lived experience
Complete program documentation to enable assessment of program experience and tracking of outcomes for site social and wellness programs
Monitor program occupancy and engagement against target outputs
Identify opportunities for improved engagement and program promotion and uptake

Essential Criteria

Appropriately trained aged care workers with relevant experience.
The following qualifications are highly recommended:
- Cert IV Leisure & Health
- Diploma of Community Services (Case Management) or
- Diploma of Ageing Studies and services
An empathy and understanding of issues for older people their families and carers and a commitment to supporting older people to remain at home in the community
Effective written and verbal communication skills
Computer literacy - MS Office Suite and electronic client care systems
Current SA drivers license
