Director of Central Intake

Job Type: Salaried Exempt
The Director of Central Intake oversees all day-to-day operations of the Central Intake department. The director acts as a resource to clinical operations and business development teams to facilitate timely and appropriate patient admissions. The Director of Central Intake Utilize clinical operational sales and customer service knowledge to manage the acceptance of supporting documentation to help determine appropriate eligibility of hospice referrals.
Key Responsibilities:
- Gather and review required documentation for the hospice admission process.
- Ensure patients referred for hospice services are evaluated for clinical eligibility receive full explanation of the Medicare hospice benefit and receive timely transition.
- Evaluate patients for hospice appropriateness which includes review of medical records and collaboration with hospice consultants and referral sources as well as collaborating with patients and family. Clinical intake Coordinator note entered regarding findings.
- Ensure orders for admission are clearly and appropriately documented in patient record.
- Obtain and enter verbal orders from physician and mid-level practitioners: ensure quality coordination and timely entry into HCHB.
- Assist with obtaining agreement for admission from referred patients and their care givers who meet admission criteria.
- Build effective referral source relationships to support company quality and responsiveness expectations.
- Maintain clinical and professional competencies.
- Perform analysis of admission process structures audit results and on-site observations.
- Recommend efficient admission solutions with regard to responsiveness to referral and transition of potential patients to admission.
Key Qualifications:
- Must possess a valid state driver license.
- Must maintain automobile insurance as required by law.
- Must maintain nursing license and must be in good standing
- Excellent verbal and written communication skills
- Must have basic demonstrated technology skills including operation of a mobile device.
- Previous experience in hospice preferred but not required.
- Must be able to drive an automobile in all types of weather conditions.
- Demonstrate ability to supervise and direct professional and administrative personnel
- Demonstrates good communications skills negotiation skills and public relation skills.
- Effective social interaction communication and organizational skills and the ability to perform a variety of tasks efficiently.
