Who we are looking for
We are looking for a compassionate and skilled Case Manager to join the Local Link pilot program supporting victim-survivors of family domestic and sexual violence in the Northern Metro region. Youll work closely with GPs community services and support networks to provide trauma-informed culturally safe case management and strengthen local referral pathways.
To be successful in this role you will:
Tertiary qualification (AQF level 7) in social work psychology counselling allied health nursing or related discipline.
Demonstrated experience in providing case management client support or service navigation within health community or social service settings.
Strong understanding of family domestic and sexual violence (FDSV) and child sexual abuse (CSA) including impacts on victim-survivors and their families.
It would be a bonus if you:
Knowledge of local service systems referral pathways and community resources in the Northern Metro region.
Previous experience as a Nurse Allied Health Practitioner or other health professional within the primary care sector.
What you will be doing
Youll act as a vital link between primary care providers RASA and specialist servicesstrengthening referral pathways and promoting integrated holistic care for those affected by FDSV. You will work in partnership with local primary health care services (PCHS) in the Northern Metropolitan region and youll provide short-term trauma-informed case management and service navigation. Your role includes conducting risk and safety assessments developing safety plans and coordinating referrals to ensure timely culturally safe and client-centered support.
