Job Summary

Position Title: Social Worker (LCSW) Emergency Department
Location: Hyannis MA 02601
Job Type: Contract 13 Weeks
Shift: Evening (3:00 PM 11:00 PM or 4:00 PM 12:00 AM) 24 Hours/Week Rotating Evenings Every Other Weekend Rotating Holidays
Pay Rate:$50 - $55/hour (Approx. $1650 weekly)

Position Summary:

Navitas Healthcare LLC is seeking Social Worker (LCSW) Emergency Departmentfor an exciting job in Hyannis MA.

Responsibilities:

Conduct psychosocial assessments for patients presenting with behavioral health substance use trauma or social concerns.
Deliver crisis intervention emotional support and short-term counseling to patients and families.
Collaborate with physicians nurses and other healthcare professionals to develop and implement care and discharge plans.
Coordinate referrals to community resources shelters and social service agencies.
Complete mandated reports (child/elder abuse domestic violence) per state and federal requirements.
Accurately document all assessments interventions and plans in the EMR.
Participate in interdisciplinary rounds care conferences and quality improvement initiatives.
Educate patients and families on coping strategies available resources and follow-up care.
Maintain compliance with hospital policies HIPAA and professional practice standards.

Qualifications:

Active Massachusetts LCSW or LICSW required.
BLS Certification (AHA) required.
Crisis Prevention Intervention (CPI) certification preferred.
Minimum 1 year of recent hospital-based or Emergency Department social work experience preferred.
Masters Degree in Social Work (MSW) from an accredited institution.
Strong knowledge of community resources crisis intervention and case management.
Excellent communication organizational and critical thinking skills.
