The Care Specialist supports the Social Recreation service line within the Disability Services division helping individuals with developmental disabilities build community inclusion and socialization skills. This role manages the full service lifecyclefrom assessment to ongoing supportensuring that each participants goals are person-centered and outcomes are measurable. Partnering closely with families Direct Support Professionals (DSPs) Regional Center staff and internal teams the Care Specialist ensures high-quality compliant and impactful service delivery that aligns with 24 Hour Home Cares mission and core values.
Primary Responsibilities
Conduct assessments develop Individual Service Plans (ISPs) and facilitate goal reviews and planning meetings for assigned caseloads.
Lead weekly training sessions for Direct Support Professionals to strengthen skills in promoting community inclusion and social development.
Manage a large caseload of families from referral to service start ensuring timely communication and a smooth onboarding experience.
Use person-centered planning techniques to establish goals and track measurable outcomes updating progress reports and service plans as needed.
Collaborate with Regional Center staff and internal departments to coordinate services and resolve issues efficiently.
Identify high-risk situations and work with leadership to develop proactive person-centered risk mitigation plans.
Maintain accurate and up-to-date documentation in the electronic case management system.
Conduct provider evaluations offering constructive feedback and recognition for strong performance.
Serve as a subject matter expert for Social Recreation authorizations and collaborate with billing compliance and partnerships teams as needed.
Support community outreach and presentations to promote awareness of Social Recreation services.
This is a hybrid position coming into the El Segundo office 1x per month.
WHAT YOU BRING TO THE TABLE:
Skills
Strong communication and relationship-building skills with families providers and Regional Center partners
Knowledge of person-centered planning and experience developing individualized care or service plans
Excellent organizational administrative and data management skills
Proficiency in Microsoft Office and case management systems
Analytical thinking with the ability to identify trends and recommend process improvements
Sound judgment problem-solving and decision-making in dynamic environments
Bilingual Spanish preferred
Qualifications
Experience in person-centered practices care coordination or similar roles (minimum 3 years strongly preferred)
Familiarity with developmental disability services and Title 17 compliance
Certification or training in person-centered planning preferred
