Job Summary and Responsibilities
As a Nurse Practitioner (Palliative Care) with CommonSpirit Health at Home youll do more than assess symptoms youll bring comfort clarity and compassionate care to patients during some of the most meaningful times of their lives. This role is your chance to truly connect with patients and families offering clinical expertise while honoring their dignity and individual needs.
In collaboration with our Medical Director and interdisciplinary care team youll play a key role in evaluating ongoing eligibility for home health and hospice care. Youll deliver personalized assessments build trusting relationships and ensure our patients receive the right care in the right setting at the right time.
What Youll Do
- Conduct face-to-face assessments to determine continued eligibility for home health or hospice care
- Create and revise individualized care plans based on patient needs
- Communicate changes in patient condition to care teams and attending physicians
- Provide education and emotional support to patients caregivers and families
- Collaborate with other disciplines and community resources to ensure comprehensive care
- Deliver services across various settings including patient homes offices and facilities
- Participate in case conferences and support paraprofessional staff
- Complete documentation timely and accurately
- Handle emergencies with sound judgment and compassion
- Help shape educational programs across the age spectrum

Job Requirements
- Masters Degree in Nursing from an accredited school
- Palliative Care Experience
- Active Nurse Practitioner license in the state of service
- Minimum 1 year of nursing experience (hospital experience preferred)
- Current CPR certification
- Valid drivers license and auto insurance (required for travel)
- Commitment to HIPAA compliance and patient confidentiality
