Job Summary:

We are seeking a dedicated and compassionate Physical Therapist to provide high-quality rehabilitation services in a long-term care setting. The ideal candidate will deliver patient-centered therapy promote functional independence and collaborate with an interdisciplinary healthcare team to enhance patient outcomes.

Key Responsibilities:

Evaluate patients functional abilities and develop individualized treatment plans.
Implement evidence-based therapeutic interventions to improve mobility strength and balance.
Document all patient care activities and progress accurately and timely.
Collaborate with nursing occupational therapy and medical staff to ensure coordinated care.
Educate patients and families on home exercise programs and preventive strategies.
Adhere to facility and professional standards of practice and ethics.

Qualifications:

Education: Graduate of an accredited Physical Therapy program.
License: Active New York State Physical Therapy License required.
Experience: Minimum 2 years of clinical experience (skilled nursing or long-term care preferred).
Travel Experience: Not required; prior travel experience preferred.
Certifications: Current BLS certification recommended.

Schedule Details:

Shift: Days (8 hours per day)
Schedule: Monday through Friday (no weekends)
Contract: 13 weeks
