Job Summary

Description:

Seeking a dedicated Case Manager to oversee a caseload within the client operations team. This role focuses on ensuring seamless service delivery providing family and provider support and maintaining strong communication throughout all stages of care.

Responsibilities:

- Maintain ongoing communication and build trust with families.

- Support and guide providers to ensure optimal client care.

- Address and resolve service-related issues promptly.

- Facilitate insurance authorizations and monitor utilization.

- Collaborate with recruiters for staffing and onboarding.

- Maintain accurate documentation and ensure database updates.

- Strong organizational and documentation skills.

- Excellent communication and interpersonal abilities.
