Looking for a compassionate LPN to service our Home Care patients in the Muskegon MI Area.

Position Overview:

As a Home Health LPN at Trinity Health At Home youll provide skilled nursing care to the organizations clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

Position Highlights:

• Effectively and efficiently plans visit content and schedule based on client capabilities condition preferences (when possible) physician orders and available resources.

• Maintains a working knowledge of applicable Federal. State and local laws and regulations the Organizational Integrity Program Code of Ethics as well as agency policies and procedures and adheres in a manner that reflects honest ethical and professional behavior.

• Performs specific treatments and administers medications to implement the medical and nursing care plans.

• Apply appropriate preventive therapeutic and rehabilitative nursing procedures and techniques to clients.

Minimum Qualifications:

• Graduate of an accredited or state approved school of practical nursing

• Licensure as a LPN in the State of Michigan

• A minimum of one (1) years of experience in an acute care setting; home care experience preferred.

• Phlebotomy and wound care experience .

• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.

• Must possess interpersonal skills sufficiently to interact effectively with clients and their families as well as all interdisciplinary caregivers peers subordinates and supervisors.

• Must have current Drivers license and reliable transportation to and from worksite.
