Bachelors degree in social work required
Masters degree in social work preferred
3 years Managed care and/or clinical care experience required
1 year health coach preferred
LBSW - Licensed Bachelor Social Worker upon hire required or
LMSW Licensed Master Social Worker upon hire required and
CCM Certified Case Manager within 2 years of hire preferred
ACM Accredited Case Manager within 2 years of hire preferred
What You Will Do
Provide individuals families and caregivers with the psychosocial support needed to cope with chronic acute or terminal illnesses including transitions to end-of-life. Services include advising family caregivers providing patient education and counseling and making referrals for other services. Facilitates discussions with physicians and other caregivers to ensure treatment and decisions are consistent with the patients individualized care plan. May also provide care and case management or interventions designed to promote health prevent disease and address barriers to access to healthcare.
- Collaborates with other professionals to evaluate patients medical and physical condition to assess clients needs and develop a plan of care. Modify treatment plans to comply with changes in clients status.
- Assess each patients unique condition combining data from medical diagnoses personal interviews and professional knowledge to establish accurate picture of overall mental and emotional health.
- Assess the biopsychosocial status of identified patients in the home or other setting and initiating appropriate community referrals including provision of nutrition prescription assistance transportation housing and/or rent assistance and in-home custodial care.
- Provide necessary support for each patient according to health status personal strengths and weaknesses emotional state and additional factors. Provides supportive counseling services to patients families and caregivers. Promote positive behavioral therapy and coping methods.
- Monitor evaluate and record client progress according to measurable goals described in the treatment and care plan.
- Counseling on end-of-life issue and assistance with advance directives.
