Your Opportunity

Navig8 supports young people aged 14-25 as they transition from state care into independent adulthood. This is an on-site position based in Bunbury and services the South West and Great Southern regions. This role is ideal for someone ready to walk alongside young people during an important turning point of their lives - offering structure practical guidance and support where it counts.

As a Case Manager you will provide hands-on tailored support to help young people build life skills navigate challenges and connect with the right services to thrive independently. Your ability to build trust offer grounded advice and advocate effectively will play a pivotal role in helping them take ownership of their future and break cycles of disadvantage.

Your Key Responsibilities

  • Respond to referrals from internal and external services conducting phone and face-to-face assessments to determine suitability.
  • Induct clients into the program clearly explaining service expectations rights and responsibilities.
  • Deliver structured person-centred case management that keeps young people progressing toward independence.
  • Facilitate regular case management sessions adapting support to individual cultural needs and strengths.
  • Assist clients in developing life skills accessing housing education and employment pathways.
  • Provide flexible outreach support in safe and familiar environments to build connection and engagement.
  • Maintain clear confidential case records in line with program and privacy requirements.
  • Strengthen networks and partnerships with government community and Indigenous organisations to support holistic outcomes.

Requirementsfor Success

  • A relevant degree with significant case management experience in youth work social work or community services.
  • Demonstrated experience managing a caseload of young people with complex needs.
  • Strong interpersonal skills to build trust maintain boundaries and guide young people with empathy and confidence.
  • Knowledge of trauma-informed practice and cultural awareness particularly around Aboriginal culture and community.
  • Sound judgement and initiative in problem-solving and decision-making.
  • Strong communication skills - both written and verbal - for reporting planning and record keeping.
  • Proven time management skills with the ability to stay organised under pressure.
  • High computer proficiency including Microsoft Office and case management platforms.
  • Must have a Full driver licence with no restrictions.

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
