Are you passionate about supporting at-risk youth and strengthening families Hope for Youth is looking for a dedicated and dynamic Case Planner to join our Nassau County PINS Diversion Program!

About the PINS Diversion Program:
Our Person in Need of Supervision (PINS) Diversion Program aims to prevent court involvement and out-of-home placements for youth under 18 by providing short-term family-centered in-home support throughout Nassau County. We focus on building problem-solving skills and linking families to long-term community resources.
Your Role as a Case Planner:
As a Case Planner you will support youth and families by improving family dynamics encouraging behavioral change and reducing the risk of placement outside the home. This role requires flexible hours including evenings and weekends and a field-based presence using your own vehicle.
Key Responsibilities:
- Carry a caseload of 1525 families
- Conduct weekly in-home visits with youth and families
- Deliver life coaching parent skills training and case management services
- Attend school meetings court appearances and service planning sessions
- Complete and update CANS-NY assessments
- Maintain accurate timely documentation and progress reports
- Participate in team meetings supervision and ongoing staff development
- Adhere to HFY and regulatory standards and practices
Qualifications:
- BSW or Bachelors in a related field with 2 years of casework experience or MSW
- Strong written verbal and tech skills
- Bilingual (Spanish/English) preferred
- Valid NYS Drivers License & access to reliable transportation
- Ability to work a flexible schedule to accommodate families
- Subject to all required background checks and clearances
Physically able to perform duties (e.g. travel climb stairs prolonged periods of walking/sitting)
