Position: Alternative Family Living (AFL) Provider
Hours: 24 hours a day 7 days a week
Compensation:Contracted Rate
Duties Include: Providing training supports and supervision to an individual with intellectual and developmental disabilities within a private home setting and in the community. AFL providers have a 24 hour 7 days a week responsibility for the individual when the individual is not engaged in other services such as a day program. AFL providers must be available to care for the individual if they are unable to attend the day program for any reason.
Responsibilities:
- To provide assistance and training for assigned consumers according to his/her Individualized Support Plan.
- To assist the consumer in functioning as successfully and independently as possible in the home family and community and document their progress towards all established goals in the home and community.
- To accurately complete all documentation required for the service at the time the service is provided.
- To communicate with their supervisor through verbal skills and written documentation regarding any contact with case managers other service providers day programs and/or MCOs.
- To report any incidents/accidents to supervisor immediately and to complete incident/accident form immediately and submit reports to the MCO Division of MH/DD/SAS and LIFE Inc. QA/QI Director
- To participate as an interdisciplinary team member.
- To maintain certifications in First Aid/CPR NCI and Medication Administration along with any other required agency MCOor State trainings.
- To assist consumers in activities of daily living (hygiene tasks eating dressing cooking all domestic chores in the home yard care clothing care etc.) encouraging the consumer to do as much for themselves as possible and concurrently document all client progress/regression towards goals.
- To serve as a crisis responder.
- To treat all clients with dignity and respect and report any abuse and/or neglect.
- Maintain a safe and clean home to ensure that client lives as normally as possible.
- Provides three nutritious meals per day as well as necessary snacks and provides a menu appropriate for any medical needs the client has.
- Provides client transportation as needed to all medical social and treatment appointments.
- Maintains all required automobile liability insurance on any vehicle while transporting client.
- Conducts and documents all required drills and evacuation processes and constantly evaluates the efficiency of those drills and processes.
- Maintains the appropriate homeowners and renters insurance and provide to the agency.
QUALIFICATIONS:
- Minimum of a high school diploma or GED.
- Valid North Carolina Drivers License and insurance.
- Must exhibit ability to read and write detailed notes to follow complex verbal and written instructions.
- Must be able to be reached by phone.
- Must be certified in First Aid/CPR Mandt and Medication Administration
