JOB DUTIES / KNOWLEDGE

1. Following the plan of care or service plan as outline by the supervising nurse.
2. Assist the patient with transfer ambulating and prescribed exercise and instructed by the Home Care Nurse.
3. Prepare modified diet as ordered by the Home Care.
4. Observe and report specific vital signs and symptoms as instructed by Home Care.
5. Remind the patient to take medications daily.
6. Monitor skin changes daily monitor changes and report to Agency RN.
7. Perform incidental household functions necessary to maintain a safe E.g. comfort cleanliness of the patient and grocery shopping.
8. Report changes in the patients physical status to the Home Care Nurse or Home Care.
9. Submit annual mandatory compliance documents such as annual TB questionnaire.
10. Completes 12 hours of mandatory annual compliance training.

QUALIFICATIONS

1. Must be a certified Home maker/Home Health Aide.
2. Be a minimum of 18 years of age able to read and write English.
3. Bi-lingual is a plus.
4. Have had a physical examination satisfying the New Jersey State Department of Consumers Affairs requirements for Home Health Care Workers.

Pay: $18.00 per hour
