The Role:

You will be working as one of two Clinical Care Coordinators (resident ratio: 1:40) and reporting directly to the Resident Services Manager you will be responsible for the delivery of resident focused clinical care services including care planning review and assessment of all aspects of clinical care.

Key Responsibilities:

  • Ensure comprehensive clinical care is provided documented and tailored to each residents needs
  • Investigate incidents daily particularly Serious Incident Response Scheme reporting and seek continuous improvement opportunities
  • Prioritise residents and their families in decision-making considering their preferences independence dignity and identified care-related risks
  • Work collaboratively with Resident services Manager and peers in Implementing Strengthened Aged Care Quality Standards
  • Oversee and conduct all scheduled AN-ACC reappraisals including mandatory and voluntary assessments as required
  • Lead the clinical team including RNs and Ens for daily case reporting care planning and addressing patient concerns promptly
  • Fortnightly report to Clinical and Quality Committee Meeting on care quality SIRS and clinical risks
  • Report quality indicators

What you bring:

  • Current AHPRA Registered Nurse DIV 1 registration with no conditions.
  • Dedication to resident care and support.
  • Excellent clinical knowledge regarding medical conditions palliative care and care of people living with dementia including
    managing challenging behaviours.
  • Advanced clinical skills and experience in managing outbreaks and emergency situations with proven ability to motivate and lead staff promoting a positive work environment.
  • Highly developed interpersonal and written skills and the ability to effectively communicate with residents resident representatives and other health professionals.
  • Strong planning time management and organisational skills.
  • Ability to complete reporting and conduct trend analysis and observations to inform best practice care.
  • A good understanding of the Aged Care Act and its principles Aged Care Standards and AN-ACC funding and palliative care funding.
