Our Centre for Seniors and Rehabilitation offers a team of health care professionals who provide standardized comprehensive geriatric assessments and care plans to support seniors with complex care needs. . This position reports to the Clinical Service Manager of Seniors Health with oversight from the Clinical Director of the Seniors program. This position will require the successful candidate to collaborate with all members of the Seniors clinics team Geriatric Outreach Team and our community partners
 
The Registered Nurse (RN) is a regulated health care professional who is accountable and responsible for providing quality patient focused nursing assessments in the care to patients within the ambulatory clinics. As part of the inter-professional health care team the RN will practice according to the nursing practice expectations set by Osler the regulatory practice standards of the College of Nurses and the legislative requirements set by the Regulated Health Professions Act.

Accountabilities:

  • You will work collaboratively with interprofessional team to provide standardized comprehensive geriatric assessments and care plans to support seniors with complex care needs.
  • You will support the ambulatory clinics  by attending education related to the model of care
  • You will inspire and lead others to effectively implement comprehensive assessments focused on managing behaviours associated with dementia and/or BPSD  through role modelling coaching and through communication
  • You will participate in change methodology to support and sustain the change to an emotion-based model of care
  • You will incorporate your knowledge and support seniors at high risk of injury due to a fall. Assessments include interviews questionnaires standardized fall risk/frailty testing physical exams and cognitive screening.
  • Safe nursing practice in all aspects of the nursing process: assessment planning implementation and evaluation of patient care
  • Facilitate access to our Seniors Clinics and Geriatric Outreach team
  • Effectively provide health teaching and support over the phone in the areas of geriatric syndromes dementia and delirium education caregiver support and linkages to community supports
  • Facilitate and/or manage ad hoc calls from patients/families requiring clinical advice
  • Ensure accurate maintenance of wait times for patients to various clinics
  • Ensure compliance with applicable PHIPPA CPSO guidelines and hospital privacy and security policies
  • Develop a process of data collection verification data entry and statistical reports
  • Check and audit geriatric charts and database for accuracy
  • Advance  a culture that celebrates the spirit of discovery  learning and innovation inspired by the diverse communities we serve to help transform to an academic health centre.

Qualifications :

  • Current registration and in good standing with the College of Nurses of Ontario
  • BCLS required
  • Experience working with geriatric and dementia patients in an acute care and / or ambulatory setting within the last two years required
  • Geriatric certificate enrollment required
  • Gentle persuasion approach required
  • Experience within the last five years with a minimum of two years in acute care setting
  • Bachelor of Science in Nursing (BScN) preferred
  • PIECES enrollment preferred
  • NICHE geriatric certificate preferred
  • Sound knowledge of nursing process theory and responsibilities to the nursing profession
  • Comprehensive understanding of geriatric syndromes delirium and dementia
  • A passion for leading dementia care
  • Up-to-date and evidenced-based knowledge and skill in dementia care
  • Your priority is the wellbeing of those patients on the unit not only tasks
  • You believe that emotions matter most


Core competencies:

  • Demonstrated experience in care of patients with enteral feeds tracheostomy wound care IV therapy blood collection and isolation
  • Demonstrated excellent interpersonal communication problem-solving conflict-resolution and decision-making skills required
  • Demonstrated ability to work collaboratively as a member of an inter-professional team
  • Demonstrated knowledge of patient and family-centred care
  • Demonstrated experience with patient and family health teaching required
  • Demonstrated leadership and facilitation skills required
  • Demonstrated excellent customer service organizational and critical-thinking skills required
  • Demonstrated commitment to continuous learning and professional development as it applies to the profession and related field
  • Demonstrated ability to work in a stressful and fast-paced environment
  • Exemplary clinical practice within an inter-professional team to develop appropriate strategies for patient care
  • Ability to prioritize patient care needs and make autonomous decisions
  • Excellent physical assessment skills required
  • Ability to promote a healthy work environment and practice excellent client safety guidelines
  • Knowledge and understanding of the Occupational Health and Safety Act
  • Excellent attendance punctuality and work record
  • Knowledge and understanding of information systems and technology

  • May be required to travel and work between all Osler sites

Additional Information :

Hours: Currently Days 8  shifts  excluding  weekends and statutory holidays (subject to change in accordance with operational requirements)

Salary:
Minimum: $40.24 per hour
Maximum: $57.68 per hour
