Fraser is seeking a Medicaid Access Coordinator to help individuals and families navigate the Medicaid this role you will provide education support and guidance throughout the application and enrollment process. As a key advocate the Medicaid Access Coordinator works to reduce barriers to coverage and ensures clients maintain ongoing eligibility.

Responsibilities:

  • Assist individuals in obtaining needed services across mental health social educational health recreational and related areas to improve overall functioning.

  • Proactively identify and address barriers to accessing recommended services.

  • Guide clients through the enrollment and renewal processes for Medicaid CHIP Marketplace or employer-based health plans.

  • Support clients with completing applications gathering required documentation and submitting necessary forms.

  • Educate clients on benefit options program rules and eligibility requirements.

  • Provide services in a culturally responsive manner recognizing the unique cultural medical psychiatric and physical needs of each individual.

  • Accurately document client-related data Social Determinants of Health and referral information in Frasers Electronic Health Record.

  • Verify required documentation such as proof of income residency and identity.

  • Collaborate with internal and external providers to facilitate consultation and ensure individuals and families have access to needed support.

Qualifications:

  • Must be qualified in at least one of the following ways:
    • High School diploma or equivalent required with one year of relevant work experience health care mental health customer service administrative experience.
    • Bachelors degree or higher in one of the behavioral sciences or related fields from an accredited college or university
    • Must be eligible and willing to complete 20 hours of online training and pass knowledge assessments
  • If required to drive the following requirements must be met:
    • Possession of a valid Minnesota drivers license and maintenance of a satisfactory driving record consistent with Fraser policy required by specific needs of the individuals receiving services.

Location Schedule & Pay:

  • Fraser Bloomington MN
    • Hybrid / remote
  • Monday - Friday working either 8 am - 4 pm or 9 am - 5 pm - full time working 40 hours a week
  • The starting pay range for this role is $23.07 - $24.00 an hour depending on qualifications.
