Empath Home Health a member of Empath Health is seeking a compassionate full-time Home Health RN to provide skilled nursing care for patients in their homes throughout East Pasco county (Wesley Chapel Land O Lakes Odessa Dade City Zephyrhills) & North Hillsborough county (Lutz New Tampa Tampa Palms Temple Terrace University and surrounding areas). In this full-time role youll deliver individualized care build trusted patient relationships and help people manage their health with dignity and independence.

Locations: Home health visits in east Pasco county (Wesley Chapel Land O Lakes Odessa Dade City Zephyrhills) and north Hillsborough county including: Lutz New Tampa Tampa Palms Temple Terrace University)

Schedule: M-F 40 hours. Required weekend/on-call/holiday rotations.

What Youll Do

• Perform comprehensive patient assessments including OASIS assessments at required intervals
• Develop initiate and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless holistic care

Position Requirements

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience.
• Requires prior home health RN experience
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment critical thinking and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
