Job Summary

Job Summary:

The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness within a home health setting. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. The RN is expected to manage an average of 5-7 patients per day meeting a minimum of 30 points weekly.
Location:Lacey Washington United States
Responsibilities:

- Conduct patient assessments in a home health setting.
- Administer medications and treatments according to physician orders.
- Develop and implement patient care plans.
- Collaborate with an interdisciplinary team to coordinate patient care.
- Provide timely and accurate documentation using HCHB EMR.
- Travel within a 60-mile radius from the hub/office to visit patients.
- Maintain open communication with patients providers and team members.
Required Skills & Certifications:

- Active Registered Nurse license.
- BLS certification.
- Minimum of 1 year of recent Home Health experience.
- Proficiency in clinical assessment medication administration and treatment planning.
- Experience with HCHB EMR.
- Valid drivers license car insurance and reliable transportation for field work.
Preferred Skills & Certifications:

- Bachelors degree in Nursing or related field.
Special Considerations:

- On-call responsibilities required (details to be discussed during interview).
- Traveler expected to be flexible with territory assignments.
- Timely documentation and communication are essential.
Scheduling:
- Guaranteed 40 hours per week.
