Job Description:
We are seeking an innovative and experienced leader to join our team as a Patient Care Manager. This role is essential in ensuring high-quality patient care in a home health setting while guiding and supporting the nursing team.

Responsibilities:

Lead and oversee patient care operations
Supervise mentor and support nursing staff
Collaborate on the development of personalized care plans
Ensure accurate and timely documentation
Audit records for quality assurance and compliance
Facilitate communication across care teams for coordinated patient care
Engage in quality improvement initiatives and staff professional development

Qualifications:

Associates degree or equivalent in Nursing
Registered Nurse (RN) licensure in the practicing state
1 2 years of management or supervisory experience in a healthcare or home care setting preferred
Knowledge of professional standards regulatory requirements and Medicare Conditions of Participation
